Updates and fixes to the Single Question Survey Box

This is the box that contains a single question from a survey, normally seen in the right column currently but also seen on the homepage.

Single Question Survey Box

1. 2 double gray 1px lines by 300px will be at the top and the bottom of the survey question
2. a 300px x 20 px hex#:e7e7e7 gray title bar will be under the double lines
3. this title bar will have the text in hex#:444444 Channel Health Guru Survey (Baby Health Guru Survey, Pregnancy Health Guru Survey, etc)
4. The survey question will be in a larger black font centered underneath the title bar
5. the answer selections are justified to the left in our light gray color (hex#:828282)
6. 'From the How Big is my Baby Survey' will be centered at the bottom of the box
7. The box will grow and shrink depending on the # of questions or how many lines the survey title wraps on to.

*example of this new design box is attached to the ticket(RtHandColSurveyBox?.jp)--we are changing this because the double lines are more consistent with the overall design rather than having it in a box.

3. Submit btn is centered underneath the answers

Added a check in the single question partial view (partials/singlequestion.phtml) which uses the variable $this->homepage to establish if the homepage or right column version of the view should be displayed.

The homepage only passes the variable homepage=true to the partial (index.phtml). All other calls to the partial pass no parameters so they render the default (else) version of the partial.
