The 300x600 Ad Unit (wideskyscraperadbox.phtml partial) should be added to all Survey pages.

added the wideskyscraperadbox.phtml partial to the rightcols of all survey pages (applications_survey.phtml, applications_surveys.phtml, and applications_surveyresults.phtml)
