Branch Surveys are breaking on SQL logic

Description

Branching surveys are generating a MySQL error when the user answers questions.

specific example, breaking after question 1, then again after question 2:

Added a third layer check that finds the next question even if there's not enough data. Doesn't work with branches but provides a fail-safe.

Will generate another bug ticket for branching case.
