CHG Survey Issues

1. Question image appears to beresized on page to wrong size

2. The profile fields box does not dissapear after answering question 1 (see example 1)

3. Answetr image appears too high and often covers question text (see example 1)

4. No skip question link on any of the questions

5. Charts on the results page do not have colors in the legend.
