Create a Back-Office tool for the creation of tree (branching) surveys and quizzes with scores.

Build a back-office tool that cleanly and clearly allows the creation of surveys that utilize branching and scoring. Branching should occur on specific responses (i.e. a different question may be asked based on the response to a question) and each response may be assigned a score (default is 0). This utility should provide the interface for developing projects such as ticket #30.

*Test Method / Usage

1. Go to Manage Surveys tab.
2. Click Edit Surveys tab.
3. Click New Survey. Create a new survey as normal, set type to quiz.
4. From the main Edit Surveys section, click 'Link Questions' for the new quiz.
5. Use the questions list to select a root question.
6. In the tree view, click on either the question or the responses to link question(s) to the tree. Different questions linked to responses will cause branching.
7. Click on a response to provide values and/or reasons for that response. Values are used for scoring the quiz while reasons are used for providing later information regarding that response.
8. When done, return to Manage Surveys.
9. Click on the Question Co-Reg tab.
10. Choose the question you wish to link information to from the drop down list.
11. For the TSTD survey, choose Text from the CoReg? type, provide the 'Did you know' fact into the Content section, and set the source to tstd.

This completes the construction of the TSTD specific quiz type.
