BHG: Survey Page

Build the Survey page for Baby Health Guru as per design.

Back end is part of new Standard platform (applications/survey)
Front end is two tasks:
1. CSS generic for Standard platform
2. CSS specific for BHG

- Retake flag is passed to survey results page if user goes to a completed survey.

-double 1px horizontal line spanning btwn the answer and the next question (this should be as wide as title bar)
-the Baby Health Guru Surveys below the reg box in the rt hand col: the survey titles and status of surveys needs to be justified to the left so it lines up with the left side of the purple bar
-survey titles in this box do not wrap..if they exceed one line, pls add '...' at the end

Made a few front-end fixes, ready for QA

Fixed error with ResponderModel? improperly restoring taken survey data into session.

Fixed error with ResponderModel? improperly restoring taken survey data into session.

The problem was related to the getCompletedSurvey method, it was not using the survey id properly in the query to hg_user_survey, the table that warehouses completion information on a per user / per survey level. The fetch was only requesting the user id due to a misformed but syntactically correct SQL call. This SQL statement has now been corrected and additionally a check is now being made to verify that the survey id matches the current one before loading the session with data.

On the first page of the survey – we do not use the image which represents the survey – was this intentional?

*** On the survey answer page – sometimes the image for the previous answer is overlapping the next question…

Upon completion of a survey – the designation that says “completed” with the checkbox under the list of surveys needs some formatting work – the grey highlight is not the same width as the purple header – the “Completed” seems to be justified to the top of the line, so it is not even with the rest of the text or the check box on that same line. Generally we should see if we can get some better looking pie charts… when there are not a lot of surveys, will the “what’s next” fill in with videos or something else relevant for them to do?
