Updates and Fixes for the Survey Results Page

1. the text 'what's next' only needs to appear above the actual sliding thumbs--it's currently directly underneath the title bar.
2. You Are Here box needs to have double lines above it (this is not in partials tick because on other pages, it has the double lines).
3. for the talk boxes that appear when user clicks in the text field, please refer to ticket #209--Register/Login Form--thanks!

4. please switch login and register fields so that the 'log in' column is to the left of the reg column--this is so this box layout remains consistent throughout the site.
5. left justify the 'submit' buttons on the bottom and the log in & register text on the top.

- The pagination should include the prefix "Page:" so it looks like "Page: 1 | 2 | Next >" also it is a little too close to the line above it.
