Description

Build interface pages for CHG for Survey application, specifically:

1. All Surveys Page
2. Profile Question Box
3. Survey Question Box
4. Survey Answer Box
5. Take Surveys Box
6. Survey Results Page - Sign In
7. Survey Results Page - Results

Pages should use the existing survey architecture.

Text in survey description box does not fit right http://devcollege.healthguru.com/applications/survey/srvyid/3/qid/49

Answer boxes do not have small pic displayed.

http://devcollege.healthguru.com/applications/surveyresults/srvyid/4 the results pag is incomplete and missing headings and does not give the user the option to login rather than just registering. Also the title says the word "survey" twice.

Text in survey description box does not fit right http://devcollege.healthguru.com/applications/survey/srvyid/3/qid/49<--fixed, I added some padding to the survey description box.

Answer boxes do not have small pic displayed.<---fixed, now the picture should be appearing properly
